This study estimates the number and proportion of work injuries reported to the United States Occupational Safety and Health Administration (OSHA) that can be attributed to heat exposure. These estimates contribute to the calculation of the benefits of standards, policies, and programs that reduce workplace exposure to extreme heat.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Methods<\/h3>\n\n\n\n<p>We analyzed all 2023 injury cases reported to OSHA&#8217;s Injury Tracking Application by establishments with 100 or more employees, primarily in high-hazard industries. Each injury was geocoded and matched with high-resolution weather data for the specific injury date. Using a case-crossover design, we compared heat index on each injury day (case) with matched non-injury control days for the same worker. Conditional logistic regression was applied separately for summer-only and year-round periods with a non-linear term for heat index to estimate the odds ratios for injury occurrence. We additionally examined heat-injury patterns by industry sectors and in states with\/without workplace heat standards.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Results<\/h3>\n\n\n\n<p>The odds of work injury increased non-linearly with a rising heat index: the pooled national estimate showed a clear upward trend starting around 85\u00b0F and accelerating above 90\u00b0F. Our results were consistent across nearly all industry sectors, including those that are predominantly indoors. Using a heat index of 80\u00b0F as reference, odds ratios (OR) of injuries at or above 90\u00b0F, 100\u00b0F and 110\u00b0F were 1.03 (95% confidence intervals [CI]: 1.02, 1.04), 1.10 (1.07, 1.13), and 1.20 (1.13, 1.26), respectively. At a heat index of 110\u00b0F or higher, the odds increased by 22% in states without occupational heat rules (OR=1.22; 1.15,1.29) versus 9% in states with rules (OR=1.09; 0.84, 1.41), suggesting a protective effect, although confidence intervals overlapped. Overall, we estimate 1.18% (95% empirical CI: 0.92%, 1.45%) of all injuries were attributable to heat exposure on days exceeding a heat index of 70\u00b0F.<\/p>\n\n\n\n<h3 class=\"wp-block-heading\">Conclusion<\/h3>\n\n\n\n<p>Heat exposure increases the overall risk of work injury, an effect consistent across nearly all major industries.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Background Exposure to heat leads to physiological and cognitive impairments that increase the risk of workplace injuries. Los programas de descanso, sombra, hidrataci\u00f3n e higiene (RSHH) son intervenciones prometedoras para la prevenci\u00f3n de la ERCnt. Un obst\u00e1culo para la implementaci\u00f3n de estos programas es la preocupaci\u00f3n de que la reducci\u00f3n del tiempo de trabajo asociada a estos programas pueda reducir la productividad y los ingresos. Este estudio de caso analiza el impacto econ\u00f3mico de un programa de RSHH implementado en un ingenio azucarero de Nicaragua.<\/p>\n\n\n\n<p id=\"p-3\"><strong>Acercarse<\/strong>&nbsp;Se obtuvieron datos del ingenio azucarero durante un per\u00edodo de seis a\u00f1os y cinco zafras (Zafra 1: 2017-18 a Zafra 5: 2021-2022). El 86% de los hogares con ERCnt informaron que el jefe del hogar hab\u00eda estado sin trabajo durante los \u00faltimos 6 meses o m\u00e1s, en comparaci\u00f3n con el 53% de los hogares sin ERCnt. Los hogares sin ERCnt percibieron m\u00e1s del doble de ingresos laborales en promedio que los hogares con ERCnt ($C52 835 y $C3120, respectivamente). Studies in general populations have described a positive association between high environmental temperatures and clinically assessed kidney outcomes, but there are no studies in occupational settings.<\/p>\n<p>Method: We accessed routine records of clinically diagnosed AKI (AKI-CD) and wet bulb globe temperatures (WBGT) at a large Nicaraguan sugarcane plantation and modeled the relationship between these using negative binomial regression. A rest-shade-hydration intervention was gradually enhanced during the study period, and efforts were made to increase the referral of workers with suspected AKI to healthcare.<\/p>\n<p>Results: Each 1\u00b0C WBGT was associated with an 18% (95% confidence interval [CI]: [4, 33%]) higher AKI-CD rate on the same day and a 14% (95% CI [-5, 37%]) higher rate over a week. AKI-CD rates and severity, and time between symptoms onset and diagnosis decreased during the study period, that is, with increasing rest-shade-hydration intervention. Symptoms and biochemical signs of systemic inflammation were common among AKI-CD cases.<\/p>\n<p>Discussion: Occupational heat stress, resulting from heavy work in environmental heat, was associated with a higher rate of clinically diagnosed AKI in a population at risk of CKDnt. Promoting rest-shade-hydration may have contributed to reducing AKI rates during the study period. Estos trabajadores tienen un alto riesgo de enfermedad renal cr\u00f3nica de origen no tradicional (ERCnt), debido al estr\u00e9s t\u00e9rmico severo que experimentan debido al trabajo pesado en condiciones de calor. Las intervenciones de RSH en estas poblaciones mejoran los resultados de salud renal, pero su impacto en la productividad a\u00fan no se ha examinado.<\/p>\n<p>M\u00e9todos: Accedimos a datos rutinarios de productividad de cortadores de ca\u00f1a de az\u00facar quemados (SC, N = 749) y de semilla (BCC, N = 535) observados durante cinco temporadas de cosecha con una intervenci\u00f3n creciente de RSH en un gran molino de ca\u00f1a de az\u00facar de Nicaragua. El personal del molino registr\u00f3 la temperatura de globo h\u00famedo (WBGT) en el sitio de campo cada hora y la resumi\u00f3 como una media diaria. Se utiliz\u00f3 una regresi\u00f3n lineal mixta para modelar la productividad diaria, ajustando por edad (18-29, 30-44 y &gt;45 a\u00f1os), sexo, WBGT (&lt;28, 28-29, 29-30, 30-31 y &gt;31 \u00b0C) el mismo d\u00eda y el anterior, temporada de cosecha (2017-18 a 2021-22), mes y estado de aclimataci\u00f3n (&lt;1, 1-2 y &gt;2 semanas).<\/p>\n<p>Resultados: Se observ\u00f3 una relaci\u00f3n dosis-respuesta inversa entre la productividad del SC y la WBGT en el mismo d\u00eda y en los d\u00edas anteriores, disminuyendo aproximadamente 3%\/\u00b0C WBGT. La productividad aument\u00f3 durante el per\u00edodo de estudio, es decir, coincidiendo con la ampliaci\u00f3n de la RSH, aproximadamente 19% en SC y 9% en BCC.<\/p>\n<p>Conclusi\u00f3n: Se esperaba que la productividad de los trabajadores agr\u00edcolas fuera menor en los d\u00edas m\u00e1s calurosos, lo que fortaleci\u00f3 el inter\u00e9s de todas las partes interesadas en mitigar el aumento de las temperaturas globales y su impacto. We aimed to explore the link between high temperatures and emergency kidney disease hospital admissions across 12 Texas Metropolitan Statistical Areas (MSAs) from 2004 to 2013, considering causes, age groups, and ethnic populations.<\/p>\n<p>Methods: To investigate the correlation between high temperatures and emergency hospital admissions, we utilized MSA-level hospital admission and weather data. We employed a Generalized Additive Model to calculate the association specific to each MSA, and then performed a random effects meta-analysis to estimate the overall correlation. Analyses were stratified by age groups, admission causes, and racial\/ethnic disparities. Sensitivity analysis involved lag modifications and ozone inclusion in the model.<\/p>\n<p>Results: Our analysis found that each 1 \u00b0C increase in temperature was associated with a 1.73 % (95 % CI [1.43, 2.03]) increase in hospital admissions related to all types of kidney diseases. Besides, the effect estimates varied across different age groups and specific types of kidney diseases. We observed statistically significant associations between high temperatures and emergency hospital admissions for Acute Kidney Injury (AKI) (3.34 % (95 % CI [2.86, 3.82])), Kidney Stone (1.76 % (95 % CI [0.94, 2.60])), and Urinary Tract Infections (UTI) (1.06 % (95 % CI [0.61, 1.51])). Our research findings indicate disparities in certain Metropolitan Statistical Areas (MSAs). In Austin, Houston, San Antonio, and Dallas metropolitan areas, the estimated effects are more pronounced for African Americans when compared to the White population.
